Shipping Damage Report

A Shipping Damage Report can be submitted by completing the form below. This form is for customers or partners providing service on behalf of end-customers who have purchased FuzionTek products from an authorized partner. There requisite guidelines for filing a damage claim are as follows:

  • All damage must be reported within 72 hours of receiving the package.
  • DO NOT dispose of the original packaging; it is REQUIRED for Carrier review.
  • Photos of the damage are REQUIRED for proper processing.

After you report is submitted we will send a follow-up email with instructions on how to send us the photos. FuzionTek Technical Support will then review all of the details, determine if an exchange is indeed necessary and respond with an RMA number and instructions for returning the product. Absolutely no refunds will be given; only exchanges
