Request Return Material Authorization (RMA)

Return Material Authorization (RMA) number(s) can be obtained by completing the Online RMA Request Form below. This form is for customers or partners providing service on behalf of end-customers who have purchased FuzionTek products from an authorized partner.

After the RMA form is received, FuzionTek Technical Support will review the details, determine if an RMA is necessary and respond with an RMA number and instructions for returning the product. Please note that each item will require you to submit a seperate RMA request form.
